What to expect on your first visit

During your initial appointment, you will receive a consultation to discuss your diagnosis and treatment options. In certain situations, surgery may be conducted on the same day. However, if you have a complex medical history or treatment plan, you will need to undergo an evaluation and schedule a separate appointment for treatment on another day.

Kindly provide the following information during your consultation to assist us.

Your surgical referral slip and any x-rays if applicable.

A list of medications you are presently taking.

If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

Patient Registration Forms

**URGENT:** All patients under the age of 18 must have a parent or guardian present for the initial consultation visit, surgery day, and all post-operative appointments.

Patients receiving IV anesthesia for surgery must have a preoperative consultation and physical examination. Please refrain from eating or drinking after 12 midnight the night before surgery and arrange for an adult to drive you home.

If you have any medical conditions or are taking any medications, please inform the office before your surgery.

X-Rays

If your dentist or physician has already taken x-rays, you can ask them to send them to our office. In case there is not enough time, please collect the x-rays and bring them to our office. If more x-rays are needed, we can take them at our facility.
